Please note that with the health permit there is a fee attached to it, but COA is for free.
The applications must be accompanied by the following documents:
- Certified ID Copies for Person in Charge
- Proof of Residential [Address]
- Training Certificates or Register of All Food Handlers (Kitchen ssaff)
- Proof of Pest Control and Extractor Services
- Fire Certificate from City of Johannesburg
- Zoning Certificate From Town Planning Department
- Occupancy certificate From Building Control Department
- Floor Plan of the Premises on a Scale of 1:50
Kitchen plan must have the following requirements:
- Two copies of detailed layout plans for the food premises, the plans must be drawn on scale 1:50.
- Separate Drawings and specifications of all proposed hoods/canopies should also be detailed for approval prior to the installation thereof in the food handling area.
- Health Notes and Key Indicators should be indicated on the plan eg. - POT SINK WASH.
A. STANDARDS AND REQUIREMENTS FOR FOOD PREMISES
1. THE PLAN should indicate the position of all facilities, equipment and fixtures in the preparation area:
a) 50% free floor space to be maintained in the preparation and scullery areas.
b) Hand washing facilities with hot and cold water in all preparation areas.
c) Preparation facilities for meat and vegetable to be separate.
d) Wash-up facilities for equipment with hot and cold water in the scullery.
e) Adequate drainage must be provided on the premises.
f) Chilling and freezer facilities for the storage, display, or transport of perishables.
g) Separate storage area with shelving for foodstuffs, cleaning chemicals, and equipment.
h) Separate hygienic storage area for refuse containers on the food premises.
i) Artificial illumination which permits lighting to fall on all food-handling surfaces.
j) Artificial ventilation which will facilitate the addition of adequate fresh air to the food-handling area and the effective removal of polluted or stale air from the food-handling area.
k) Refuse area and a waste-water disposal system including a grease trap.
1.1 ABLUTION/STAFF FACILITIES
a. Separate changing areas with lockers for personal items for males and females.
b. Sanitary facilities as specified in Annexure D of R638 of 22 June 2018 for the use of workers on the food premises and for use by persons to whom food is served for consumption on the food premises as well as the dining area, etc.
